Community Nursing and Treatment Rooms

Definition/Description

The Community Nursing and Treatment Room Service provides support and care for patients with a range of nursing needs including those recently discharged from hospital (including discharges out of hours), those recovering at home and those at end of life

The services will work with patients and their carers to provide advice and guidance on all aspects of their care, promote independence and help delay deterioration and reduce the risk of avoidable hospital admission.

Service Opening Times
Community Nursing

24 hours per day, 7 days per week, 365 days per year

  • 8:00 to 18:00hrs – planned and clinically essential unplanned care delivered
  • 18:00 to 20:00hrs - clinically essential unplanned care delivered
20:00 to 8:00hrs - out of hours service for clinically essential unplanned care

Monday to Friday: 08:00 – 18:00hrs (excluding Bank Holidays) –see below

Saturday / Sunday / Bank Holidays and Out of Hours urgent access and support provided by Community Nursing teams.

Clinics are staffed by Registered Nurses and Health Care Assistants across each week

Patients will be asked for preference of Treatment Room to attend, however, if no appointment is available, they may be offered appointments at an alternative Treatment Room ,subsequent appointments may then be arranged at their preferred / nearest Treatment Room.

Red Flag Symptoms

Patients with diabetic foot ulcerations should be referred immediately to:

CHCP Podiatry Service - via 01482 247111 or via NHS eReferrals – CHCP Podiatry Service

Palliative patients with uncontrolled / complex symptoms make direct referral into CHCP Specialist Palliative Care via CHCP Care Coordination Hub on 01482 247111 (option 9)

Exclude Red Flag Symptoms

For Lower Limb wound referrals immediately escalate to the relevant clinical specialist, those with the following ‘red flag’ symptoms / conditions:

  • Acute infection (e.g. increasing unilateral erythema, swelling, pain, pus, heat).
  • Symptoms of SEPSIS
  • Acute or suspected chronic limb threatening ischaemia, (e.g. PAD in combination with rest pain, gangrene, or lower limb ulceration > 2 weeks duration)
  • Suspected acute deep vein thrombosis (DVT)
  • Suspected skin cancer
  • Bleeding varicose veins

Do not refer

  • Patients under 18 years old
  • Care / treatment or screening which is not part of the Community Nursing and Treatment Room Services interventions offer
  • Treatments / investigations which are commissioned under other arrangements e.g. flu vaccinations
  • Interventions required at Nursing Homes due to skill and competency gaps
  • Patients who following risk assessment have breached the ‘CHCP Violence and Aggression Policy’
  • Patients who have multiple consecutive failed home visits (as per ‘Did Not Attend - DNA’ guidance) despite warnings sent utilising applicable policy

Referral Criteria/Information

  • Patients aged 18 years and over
  • Patient has a nursing need which meets the service intervention criteria

The Community Nursing and Treatment Room service offer a range of clinic-based appointment and home visits for housebound patients.

Referrers should be aware that all patients should be encouraged to attend a Treatment Room Community Clinic whenever possible.

At the time of the first assessment patients will be assessed for the most suitable arrangements for ongoing care using the definition below:

Community Nursing and Treatment Room Service housebound definition:

Being housebound means an individual is unable to leave their home on their own or with assistance of others due to advanced age, illness, or a disability. The characteristics of advanced age, illness or disability alone do not necessarily mean that someone is housebound.

Therefore:

• Those patients who are able to leave home independently or with assistance of others (including those using mobility aids such as wheelchairs, scooters etc.) are not ordinarily considered to be housebound.

• Patients who are able to leave home independently or with the assistance of others for social activities or other appointments (healthcare or otherwise) are not ordinarily considered to be housebound.

The Community Nursing Service will therefore assess patients based on these criteria. Those not meeting the above criteria will be booked to have their treatment and care provided in the treatment clinic and not at home.
