Appendicectomy in patients with suspicion of appendicitis

For the treatment of

Acute appendicitis

Commissioning position

This intervention is only commissioned when used in accordance with the recommendations specified in the Academy of Medical Royal Colleges (AoMRC) Evidence Based Interventions (EBI) programme: Appendicectomy without confirmation of appendicitis - EBI

Summary of rationale

Appendicitis is a common surgical emergency typically diagnosed through history and physical exam, though imaging is useful when the diagnosis is uncertain. Imaging can reduce unnecessary surgeries and identify candidates for conservative management. Ultrasound is the preferred first-line test; if results are inconclusive, CECT is used. MRI offers similar accuracy to CECT but is less accessible; however, it is preferable for younger or pregnant patients due to the absence of ionising radiation.
