Definition/Description

The Hull and East Riding Community Chronic Pain Management Service

The Hull and East Riding Community Chronic Pain Management Service offers chronic pain management education, advice and holistic interventions to patients who have symptoms of pain which have been present for more than three months, and when acute treatments, investigations and interventions by either GP or MSK (if appropriate) have been completed. We ask that patients are willing to engage in a package of care, including assessment, physical activity programme(s), our educational Pain Management Programme, plus any other holistic interventions which would be appropriate, such as psychology delivered either one to one, or as part of a yoga therapy course (including breathworks and relaxation, mindfulness etc), acupuncture, pharmacy support, pilates, manual muscle testing and re balancing (KORE MSK Therapy)

Red Flag Symptoms

Red Flag Symptoms

Any red flag symptoms to be referred to A&E

Exclude Red Flag Criteria

  • Acute pain / pain of less than 3 months duration unless there is a risk of the pain becoming chronic
  • Known pathology requiring further investigation
  • Suspected cancer or cancer related pain
  • Palliative pain
  • Pre-natal back pain / pelvic pain
  • Chronic Fatigue Syndrome (unless the primary presenting symptom is pain)
  • Service users receiving care from another chronic pain management service
  • Service users under the age of 18
  • Service users not registered with an NHS Hull or East Riding of Yorkshire CCG GP
  • Service users requiring a surgical opinion
  • An MSK condition that has not been triaged by MSK services

Guidelines on Management

Referral to MSK service for triage/treatment prior to referral to community pain if patients meet MSK criteria.

Referral Criteria/Information

Do Not Refer

When a patient has not had any suspected serious pathology excluded, have not been assessed or treat in MSK services (if problem is MSK related), for access to stand alone treatment such as acupuncture (the patient must engage with the full package of care), for repeat prescribing or for stand-alone support with drug dependency. If you have referred to another pain service for assessment and management of the same condition at the same time.

Referral Criteria

  • Patients must have a Hull or East Riding GP
  • Patients must be 18 or over
  • Patients must be experiencing chronic pain of 3 months or more (if less than 3 months there is a risk of the pain becoming chronic)
  • Patients must have a diagnosis, where possible
  • Patients have had investigations by the referring GP/Clinician, to ensure diagnosis is of chronic non-malignant pain
  • Patients are not under another team or provider for the same problem, unless considered appropriate and core to the patient's treatment plan

Other Community Health Services – via e-referral, email or S1-S1 referral

Any Other Information

Relevant diagnosis, the location of patient’s pain, length of pain symptoms, any information about a patient medication either they are taking already or have previously taken, any prior treatment a patient might have had, treatment from any other pain service, any drug seeking behaviour, summary of a patient’s present condition, list of all current mediation/allergies, copy/summary of all relevant investigations, relevant past medical history and any other specialities involved in patient care, any accessible information requirements or communications needs (including interpreter, Makaton, British Sign Language etc), patient expectations and ability to engage with a full package of care.
