Fracture Liaison Service
Definition/Description
We are a multi-disciplinary team consisting of nurses, clinical support staff and Admin support
Red Flag Symptoms
The FLS is not a primary prevention service and can only see people that have fractured.
Any immediate support for fractures refer to UTC/Fracture clinic
For parenteral treatment advice and support refer to Metabolic Bone Team
Exclude Red Flag Symptoms
Urgent response refer to fracture clinic/Emergency care
DO NOT REFER
- Under 50 years of age
- Traumatic injury
Guidelines on Management
- Identification and diagnose of Osteoporosis
- Links with other healthcare professionals and referrals on – Metabolic bone, GP’s, Hip fracture nurse, Royal Osteoporosis Society.
- Initiation on bone protection therapy
- 4- and 12-month reviews
Open access to service for advice and support.
Referral Criteria/Information
- Clients must be registered with a Hull GP or East Riding GP
- Adult 50+
- Fragility fracture
- New diagnosis of Osteoporosis

Information to Include
Please make sure the referral form is fully completed. The service will triage and if appropriate perform an assessment leading to an individual intervention plan looking to reduce future fracture risk.
