Falls Prevention Service
Definition/Description
The Hull and East Riding Integrated Fall’s Service is a Therapy and Nursing service without direct medical assessment and intervention. It is commissioned to provide therapeutic assessment and Intervention to adults over the age of 18 who are registered with a Hull or East Riding GP who have had 1 or more fall, in the past 6 months.
Red Flag Symptoms
The Falls team is not an urgent response service - refer to Intermediate care or UCR team for an urgent response
Exclude RED flag symptoms
Sufficient medical review (to be fed back to the service) r/o medical reason for fall
Urgent response consider intermediate care
Guidelines on Management
If immediate intervention is required i.e. to prevent hospital admission and requires urgent MDT assessment – clinicians should seek advice from or refer to the intermediate care team
The falls service is not a community rehabilitation service, any patient requiring rehabilitation should be referred to the community rehabilitation services.
Consider review if reason for fall is medical i.e. loss of consciousness consider referral to level 3 falls team

To reduce further risk of fall – please consider:
- Optician referral
- Advice against ill-fitting footwear
- Advice to remove loose carpets/rugs
- Complete a medication review
- Referral for lifeline
Referral Criteria/Information
Patients must be registered with a Hull or East Riding GP (except Pocklington)
Over the age of 18
One or more fall in the last 6 months
