Routine

Email/letter request to Paediatric Appointments Team: hdft.paediatricappointments@nhs.net

• Specify time interval
• If child over 8 years, need justification for reason unable to attend adult phelbotomy
• Ensure requests are on ICE
• Print form if child over 8 years and attending HDFT adult phlebotomy
• Phlebotomist will write to GP once bloods taken (or if patient was not brought)

Urgent (within 2 weeks)

• Phone consultant of the week via Woodlands Ward 01423 553670/1 to discuss and if appropriate refer via hdft.paediatricappointments@nhs.net
