Weightwise Tier 3 (Adults)

Definition/Description

The Tier 3 Specialist Weight Management Service offers appointments either by telephone, face to face in clinics across Hull and East Riding, home visits if truly housebound, video consultations and group based video education

Unintentional unexplained weight loss which requires further investigation

Guidelines on Management

This guidance is intended for clinicians referring into the Tier 3 Weight Management Service for patients:

  • Requires a Tier 3 Weight Management service.
  • Adults aged 18 years and over with a Hull or ER GP only.
  • Children from age 5 years plus with a Hull GP only

Referral Criteria/Information

Requires a referral form to be completed which are sent to chcp.247111.nhs.net.

Inclusion criteria

Adults

BMI >40.0kg/m2

OR

BMI >35.0kg/m2 plus 1 obesity related co-morbidity.

Children

BMI >98th Centile for weight (Hull GP only)

All referrals need to meet the referral criteria for the service, and consent gained for the referral.

Referrals whereby alternative weight management programmes are being followed will not be accepted.

Referrals whereby private prescriptions for weight management medications are being purchased will not be accepted.

Referrals for Weight Management injectable medications (GLP-1s) are not accepted by the service due to no commissioned pathway.

The referral form needs to be completed with all the relevant information to be accepted.

Triage Criteria

Patients who are referred into the service are triaged within one working day of receipt of referral.

The service has no priority referrals all referrals are classed as routine. KPI for wait times.

Routine: 18 weeks –

Initial assessments are usually an initial telephone assessment unless telephone is not indicated.

Whilst we appreciate waiting times can occasionally be lengthy, we cannot support letters asking to expedite referrals unless Priority criteria are met.

Exclusion Criteria and options for alternative services

Children

  • Any child that does not meet the referral criteria to enter the service.
  • Any Child with an East Riding GP

Adults

  • Any adult that does not meet the referral criteria for the service.
  • Referrals for Weight Management injectable medications (GLP-1s) are not accepted by the service due to no commissioned pathway.
  • Referrals will not be accepted if alternative weight management pathways are being followed i.e. slimming clubs, private prescriptions for weight loss medications or alternative weight management procedures.
