Tier 2 Adult Weight Management Service

Definition/Description

  • Individuals aged 18 years or over
  • Hull Resident
  • BMI 30 up to 39.9 (BMI 27.5 - Black African, African-Caribbean and Asian Origin)
  • Motivated to engage in the service.
  • The service aims to support clients to lose weight (target 5%) over an intensive 12-week programme of motivational support, nutritional education and exercise/ physical activity. The service will encourage continuation of the behaviour changes and follow up progress at 12 and 26weeks.
  • Standard Tier 2 referral form completed and sent through to - chcp.healthyhull@nhs.net
  • Self-referral accepted

Red Flag Symptoms

BP over >180/>100 will not be able to participate in a supervised exercise programme . Medical management until stable and below the upper limit to safely exercise.

No recent cardiac event – alternative support through cardiac rehabilitation.

Pregnancy – alternative referral support route via midwife.

Referral Criteria/Information

Check eligibility criteria:

  • Individuals aged 18 years or over
  • Hull resident
  • BMI 30 up to 39.9 (BMI 27.5 - Black African, African-Caribbean and Asian origin)
  • Motivated to engage in the service.

Complete standard referral form containing all required fields.

  • Consent from patient
  • Demographics
  • Height, Weight, BMI, Blood Pressure
  • Medical conditions (current)
  • Medication

Referrals

Clinical referral form to be sent to chcp.healthyhull@nhs.net or Self referrals – telephone, text or web enquiry from the patient

DO NOT REFER

  • If the patient is not ready to make change/ participate in the programme
  • BMI under 30 or over 39.9
  • Patient has an active substance/alcohol misuse or dependence
  • Patient is pregnant
  • Patient has unmanaged co-morbidities
  • Patient has an eating disorder
  • Patient has undergone bariatric surgery within the past 2 years.

Individuals in the following categories are ineligible for this service and should be referred on or signposted to alternative services.

  • Hull residents aged 17 years and under – children/ family services
  • People with an underlying medical cause for obesity who would benefit from more intensive clinical management than a tier 2 service (i.e. referral on to the Tier 3 Multi-Disciplinary Team), including those with BMI 35+ with comorbidities.
  • People who have significant co-morbidity or complex needs, as identified by their GP or other healthcare professional - referral on to the Tier 3 Multi-Disciplinary Team
  • Pregnant women -referred to Healthy Lifestyle Midwife
