Primary Care Mental Health Network (Hull)
Definition/Description
Practices can refer to PCMHN teams who can review referrals where the pathway of the support a patient requires is unclear. PCMHN teams can complete an assessment for individuals who would benefit from another, non-directly bookable person, within the PCMHN (i.e Social Worker/Clinical Associate Psychologist), or refer on to another mental health service which is more appropriate to meet the needs of the patient.
Red Flag Symptoms
Urgent Referrals which require same day response or a response within 72 hours or requiring higher level clinical interventions should be referred to the Crisis Team
Guidelines on Management
Severe presentations which are complex and with increased risk would be more suitable for Secondary Mental Health care services (MHTAT/Crisis Team).

Referral Criteria
- Please ensure the mandatory PCMHN referral form is attached
- Please ensure that the patients demographics, address, contact and next of kin details are up to date.
- Please ensure Shared record details are up to date.
- Transport requirements should be arranged by the patient if necessary.
- If Interpreter service is required please inform on the referral (specific language spoken).
Any Other Information
Routine secondary care mental health assessments should be made directly via eRS - Mental Health Triage and Assessment Service - Humber Teaching NHS Foundation Trust - RV9.
Referrals for perinatal mental health should be made directly via eRS
Referrals for NHS Talking Therapies should be made directly to NHS Talking Therapies Hull/Let's Talk
Referrals for assessment for social care requirements (non-mental health) should be made directly to the Hull Local Authority SPOC.
Referrals for drug and alcohol needs should be made directly to ReNew.
Referrals for eating disorders should be made directly to EVOLVE
