ICC - Urgent Community Response (2hr)

Definition/Description

The 2 hour Urgent Community Response (UCR) service provides urgent care to people within their own residence.  The service is available to anyone over the age of 18 and registered with either a Hull or East Riding GP.

A 2-hour response is typically required when a person is at risk of admission (or readmission) to hospital and it is likely they will attend hospital within the following 0 – 24-hour period, without intervention to prevent further deterioration and where the response can keep the person safe at home / usual place of residence. Typically, this means intervening for people living with frailty to support with assessment, diagnostics, treatments, equipment, and rehabilitation.

The service is supported by a multidisciplinary team inclusive of GPwER – Frailty, Consultant Geriatricians, Advance Clinical Practitioners, Paramedics and Clinical Support Workers.

Following initial clinical telephone triage, patients are visited and clinically assessed where necessary to do so with step up to frailty hospital at home (virtual ward) as appropriate.

Guidelines on Management

Inclusion criteria:

  • Aged 18 or over
  • Registered with a Hull or East Riding GP
  • Response needed within 2 hours to avoid a hospital admission

Exclusion criteria

  • Patient requires immediate emergency responses, e.g. life threatening, sepsis
  • Mental health crisis
  • Social care crisis without requiring medical assessment or intervention
  • Not at risk of hospital admission requiring a 2-hour response (e.g. requires same day Primary care response)

Referrals are accepted from any healthcare or social care professional involved in the care of the patient.  All referrals must be made directly by the healthcare/social care professional. 

We are unable to accept referrals made by GP administrative staff.

The service is open for referrals daily between 8am and 6pm
