ICC - Advice and Guidance

Definition/Description

The frailty advice and guidance line offers support to healthcare professionals requiring specialist advice in the management of people living with frailty, reducing the need for escalation/referral of patients to secondary care services. 

Advice and guidance provided is inclusive of but not limited to: -

  • Shared decision making and support for any person living with frailty (Clinical Frailty Score 5 and above)
  • Support which enables people living with frailty to be cared for in their preferred place of care
  • Escalation and advance care planning
  • Agreeing and mobilising a treatment plan
  • Triage for patients living with frailty including admissions to step up community beds or hospital where appropriate.

This is a small and specialist resource and is not an alternative or overflow to same day Primary Care.

Guidelines on Management

Inclusion criteria:

  • Clinical Frailty Score 5 or more
  • Registered with either a Hull or East Riding of Yorkshire GP practice

Exclusion criteria

  • Repeat prescriptions

Referral Criteria/Information

Referral sources:

  • Referrals are accepted from any healthcare or social care professional involved in the care of the patient.  All referrals must be made directly by the healthcare/social care professional. 
  • Care Home staff

We are unable to accept referrals made by GP administrative staff.

Referral criteria/Information:

The service is open for advice and guidance between Monday to Friday during the hours of  8am and 6pm.
