HNY Policy and Pathway Repository - Humber

This pathway should be used for the referral of individuals who are newly diagnosed with HIV infection, and for individuals who are known to be living with HIV who require access to care.

Any acute medical episodes will need to be referred to secondary care

Management will be in accord with BHIVA guidelines:(https://www.bhiva.org/ClinicalGuidelines) and Regional ARVT treatment algorithms and HIV MDT discussions.

Patients with known HIV infection (both newly diagnosed and established infection) who wish to access HIV care with this service. Information to Include

Please include all relevant information about this patient’s HIV diagnosis, previous care, any current or past medical conditions and any other medical treatment prescribed in primary care or elsewhere.

Self-referral: Patient who are already aware of their diagnosis and transferring from a different clinic access our service generally by phone or in person.  They are given an appointment with CNS in HIV: confirmatory tests are taken along with ‘baseline’ tests.  Patients are then offered an appointment with a Consultant.

Patients who are referred from primary care: a written letter is sent and an appointment is then offered with HIV CNS and Consultant
