Community Heart Failure Specialist Nurse Service

Definition/Description

The Hull & East Riding Community Heart Failure Specialist Nurse Team provide a high quality service that offers an individual approach to patients diagnosed with heart failure due to reduced ejection fraction (HFrEF), providing a seamless care provision between primary and secondary care.

Red Flag Symptoms

Do Not Refer

Do not refer if patient is under 18 years of age.  Do not refer if there is not a diagnosis of HFrEF.  Do not refer if the patient needs urgent attention. Do not refer patients with atrial fibrillation and fast ventricular rate or with HFrEF associated with acute MI

Guidelines on Management

If a diagnosis of heart failure is suspected, then perform a NT-pro-BNP blood test and if 1000 or above, refer to the QED (heart failure diagnostic clinic) at Castle Hill Hospital. Patients must have a diagnosis of HFrEF prior to referral to the community service.

Referral Criteria/Information

Patients must be registered with a Hull or East Riding GP

Over the age of 18

 Heart Failure with reduced ejection fraction and patient consents to referral

Diagnostic findings, dry weight if known, current medications, PMH/co-morbidities
