CDC Phlebotomy Service
Definition/Description
The Phlebotomy service at the Scunthorpe Diagnostic Centre run by the Humber Health Partnership aims are:
- To provide a rapid access phlebotomy service for routine and urgent blood samples for patients aged 12 and above
- To work collaboratively with Primary Care to ensure ease of access
- To ensure patients across North Lincolnshire are not waiting an unnecessary amount of time for blood tests
- Clinic operating times 0800-2000 pm Monday to Saturday
The service will take blood from all patients who have a valid request on Web V . Patients should not ring to book an appointment or scan QR Code until the request is confirmed as placed by the requesting clinician.
Patients that have a blood form from their requesting clinician can book an appointment and attend with their completed blood form.

DO NOT REFER
Anyone under the age of 12 years
Anyone who needs bloods via a line or port (the service is for peripheral bloods only)
Patients who are bedbound/stretcher patients should not be referred for bloods
Referral Criteria/Information
GP/Healthcare professional to complete a Web V request as 'Sample later' ensuring the requesting clinicians details are included in the request.
Once the Web V request is completed the patient is provided with booking instructions, a QR code/electronic booking or phone number to book their appointment: This can be completed by the patient, relative or a healthcare provider.

Online booking will generate an email confirmation and reminders of their appointment date/time.
Appointments will be offered at the earliest available date/time or at the request of the patient.
All results will go back to the requesting clinician electronically, as per existing process.
