0-4 Covid 19 vaccinations (immunocompromised)

Definition/Description

The aim of the COVID-19 immunisation programme is to prevent serious disease (hospitalisation and/or mortality) arising from COVID-19.  Following the introduction of vaccination post Pandemic, the risks of hospitalisation and mortality from COVID have reduced.  The Joint Committee on Vaccination and Immunisation (JCVI) have reviewed the epidemiology and noted that there are now, 2 groups who continue to be at higher risk of serious disease, including mortality.

  • adults aged 75 years and over
  • individuals aged 6 months and over who are immunosuppressed (as defined in the ‘immunosuppression’ sections of tables 3 or 4 in the COVID-19 chapter of the Green Book 2025 – pages 24-25)

 

This pathway is for the immunisation of children aged over 6 months and under 5 years of age that are immunocompromised.

 

 The service is provided in the Paediatric Outpatients Department of the Women and Children’s Hospital at Hull Royal infirmary

Guidelines on Management

As specified in the COVID-19 chapter of the Green Book (2025) and the Joint Committee on Vaccination and Immunisation (JCVI), immunisations for COVID 19 is now only available to :-

  • adults aged 75 years and over
  • residents in a care home for older adults
  • individuals aged 6 months and over who are immunosuppressed (as defined in the ‘immunosuppression’)

 

Immunocompromised children over the age of 5 years are eligible to be vaccinated through GP and Pharmacy programmes.

 

For children and young people who are immunosuppressed, JCVI continues to advise the use of the Pfizer-BioNTech COVID-19 mRNA (Comirnaty) vaccine, with the vaccine dose appropriate to the child’s age:

  • 12 to 17 years: 30 micrograms
  • 5 to 11 years: 10 micrograms
  • 6 months to 4 years: 3 micrograms

Referral Criteria/Information

DO NOT REFER

  • A child not considered to be immunocompromised
  • A child aged 5 or over who is immunocompromised – as these vaccinations can be handled in primary care.

REFERRAL CRIRERIA

Referral to secondary care for children aged over 6 months and up to 5 years of age who have a diagnosed immunocompromised system. Only referrals from a clinician will be accepted.
