IAPT - Stress and Anxiety

Definition/Description

The Hull Depression and Anxiety Service is led by CHCP CIC and provides a range of local specialist help and treatment offered by organisations including Humber NHS Foundation Trust, Cruse Bereavement Care, Mind, Relate, Focus, Joan Pickard, Psychology Online and CHCP CIC.

The service supports people with mental health problems such as mild to moderate stress (Stress Control & Cluster 1-4), anxiety and depression and bereavement.

Humber NHS Foundation Trust offer more complex secondary care services (Cluster 5 and above).

The services are coloured coded by cluster to help patientss access the correct services.

Red Flag Symptoms

If someone has taken an overdose or self-harmed and need urgent medical treatment refer to A&E or call 999. 

If a patient has urgent or more complex mental health problems contact Humber NHS Foundation Trust’s Secondary Mental Health service on 01482 336161.

If a patient has difficulties in caring for themselves or dependants, or are in need of urgent social support contact Hull City Council on 01482 300 300. In cases outside of normal office hours please contact the Emergency Duty Team on 01482 788 080.

Samaritans who will help patients to talk things through ina confidential manner. They are available 24 hours a day for 365 days per year. The local Hull number is
01482 323456 and their national number is 08457 909090. Samaritans services are free to access but call charges apply.

Guidelines on Management

The Hull Depression and Anxiety Service is a self referral service.

At the point of referral all GPs will receive a communication letter of the assessment outcome, treatment plan agreed and the provider and pathway route the patient has chosen.

Patients must undergo an assessment through the Hull Depression and Anxiety Service before they can access the services within the appropriate cluster.

The core booking service will be available 24 hours per day, 7 days per week. The assessment and clustering service will be offered from 8.00am until 8.00pm and weekends subject to patient request.

The patient will be triaged over the phone and offered an appointment for a face to face clinical assessment within 7 days of receiving the referral.

Based on the assessment, patients will be given a choice of appropriate cluster services and will be able to access them quickly. The clusters are colour coded for patient ease. The patient makes contact with the chosen service within the cluster.

Patients cannot refer themselves directly to the cluster services without having had an assessment.
