Definition/Description
These forms are only to be used for referrals with a high risk of cancer. To assist you in evaluating the potential risk for cancer the QCancer assessment can be beneficial. QCancer has been developed for the UK with risk algorithms. This tool will only provide a level of risk of cancer and is not a diagnosis tool.
Red Flag Symptoms
Diarrhoea and raised calprotectin - Stool for culture and refer to Medical Gastroenterology
- Isolated low ferritin with normal Hb.
- Individuals who have undergone colonic investigations in the last 2 years – refer routinely
- Individuals who have been referred to the 2WW system within the last year – refer routinely or contact Consultant directly
- Raised calprotectin – refer to medical Gastroenterology
- Asymptomatic individuals with family history – refer routinely
Guidelines on Management
- Patients with diarrhoea should have a stool culture and if symptoms persist a referral to medical Gastroenterology.
- Symptoms of constipation should be treated with dietary advice and OTC laxatives.
- New onset anal canal type bleeding: treat with laxatives and refer routinely if symptoms persist.
Referral Criteria/Information
Telephone Assessment Clinic prior to Diagnostic Test if:
- They are aged 40 and over with unexplained weight loss and abdominal pain or
- They are aged 50 and over with unexplained rectal bleeding or
- They are aged 60 and over with:
- Iron-deficiency anaemia or
- Changes in their bowel habit, or
- They are aged under 50 with rectal bleeding and any of the following unexplained symptoms or findings
- Abdominal pain
- Change in bowel habit
- Weight loss
- Iron deficiency anaemia
- Positive FIT Test
Out Patient Appointment
- They have a rectal mass (any age)
- They have an abdominal mass (any age)
- Anal Cancer - Refer patient using this form if:
They have an unexplained anal mass or unexplained anal ulceration

Electronic referrals can be made via the NHS e-Referral system. Services for a 2 week wait, suspected cancer are found by searching on speciality for 2WW and then selecting the clinic type i.e.. 2WW Lower GI. The referral can be made with GP system templates and please note a referral letter is no longer required with this proforma.
