Weightwise Tier 3 (Adults)

Definition/Description

Pathway to be followed should a referral for specialist weight management required for an adult or child with a Hull GP

Red Flag Symptoms

Unintentional unexplained weight loss which requires further investigation

Guidelines on Management

There are no alternative tier 3 pathways, referral required to the service

Referral Criteria/Information

Must have a Hull GP

Adult referrals (18years+)

BMI >40.0kg/m2

OR

BMI >35.0kg/m2 plus 1 obesity related co-morbidity

Note: as per NICE referrals for black and ethnic minority groups will be accepted with a 2.5kg/m2 reduction in BMI criteria to the above due to increased risk

Children Referrals age 5years-18 years (if age 4 but in school will be accepted)

BMI >98th Centile for weight.

Referral pathways

Referrals will be accepted via the following routes

  • SystmOne to SystmOne referrals

  • Referral form either via E-Referral system (RAS) or via e mail CHCP.247111@nhs.net

DO NOT REFER

  • Referrals for Weight Management injectable medications (GLP-1s, Mounjaro and Wegovy) are not accepted by the service due to no commissioned pathway.
  • Referrals will not be accepted if alternative weight management pathways are being followed ie slimming clubs, private prescriptions for weight loss medications or alternative weight management procedures.
  • Any referrals that do not meet the referral criteria below

Cross over with other ICBs/Areas

No cross over. Only commissioned for those with a Hull GP
